Registration Form

Each attendee is required to fill out a registration form completely so the number of students and classes being attended can be tracked. Photocopy the form as needed for all attendees from your agency. Please indicate whether you are registering as an individual or as part of a team. Make sure the mailing address that you list is the address where you will want to receive The Greenlight, newsletter for the MSTOA. Be sure to make class choices for all three categories. Full class descriptions (including instructors) and equipment needs will be posted on the web-site (www.mstoa.com) and in the newsletter. Classes are filled on a first come, first serve basis.             Non-MSTOA member                                  $175.00

            MSTOA member (1-5 members)                 $150.00

            MSTOA member (6 + members)                 $125.00

 Fees are $150 for per person up to 5 people and $125 for 6 or more.

After April 27, 2009, late registration fees of $50 per individual and $150 per team apply. Please get applications in prior to this to help with conference planning.

Fees include association membership for 2009 and conference tuition. Lodging and meals are not included in the fees, however, some meals may be provided (business meeting and vendor show). Base accommodations are extremely reasonable (most everything less than 25.00 per night). To make room reservations at the base, contact Training Site at (406) 324-3355 and tell them you’re with MSTOA. Please indicate your respective course choices below
